You can also apply online by following this link:

Click on the link above.
On the left hand panel choose your state.
The left hand panel will change, click on "Senior Health Plans".
A menu will open up in the left hand panel, choose "Medicare Advantage - Enroll Now".
A popup should come up, follow the directions to Enroll Online.
